Will Toenail Fungus Go Away on Its Own?

Short answer: almost never. The American Academy of Dermatology states that nail fungus almost never goes away on its own, and it can spread. Clearing an established infection usually needs prescription medication, and the nail still has to grow out afterwards.

What happens if you wait

A mild infection can sit quietly for a long time, which is why people assume it is harmless. Left untreated, the AAD notes it can cause pain and misshapen nails, make walking or standing uncomfortable, and spread to other nails or between athlete’s foot and the nails. It is contagious, though not dramatically so.

What looks like it going away, but is not

  • Growing out a damaged tip. Trimming the affected end can make the nail look better while the fungus remains at the base.
  • Cosmetic cover-up. Polish can hide colour; see nail polish and toenail fungus.
  • It was never fungus. Injury, psoriasis and other conditions mimic it: bruise or fungus, psoriasis or fungus. A bruise does clear as the nail grows out.

What actually clears it

Treatment, in order of strength of evidence, is covered in our ranking of the best toenail fungus treatments. Oral antifungals such as terbinafine have the best cure rates and need medical supervision; prescription lacquers and solutions suit milder cases. Even after successful treatment, the AAD notes it can take a year or longer for a big toe to show a clear nail. Our page on how long remedies take explains the timeline.

When not to wait

See a doctor or podiatrist promptly if you have diabetes, poor circulation or a weakened immune system, or if the nail is painful, swollen, oozing or lifting (why diabetes changes the advice). This page is general information, not medical advice.

If it keeps coming back

Recurrence is the usual frustration after treatment. There is a paid nail-care and diet guide aimed at it, Overcoming Onychomycosis. It is a support plan rather than a treatment, and we have read the publisher’s member copy but not followed it; the review covers what it claims and where the sales page oversells.

Frequently asked questions

Can toenail fungus go away without treatment?

According to the AAD, almost never. Mild cases can look stable for a long time, but they usually persist and can spread.

Is it dangerous to leave it?

For most healthy people it is mainly a cosmetic and comfort problem that can worsen. For people with diabetes or poor circulation it is a medical issue that needs a doctor early.
